Leading with the directory is the right emphasis and it addresses the question that defeats most clinical messaging. Before a message can be sent securely, the sender must establish who the recipient is and how to reach them, and in United States healthcare that is genuinely hard: providers move practices, organisations merge, and no single authoritative source of who works where is maintained. A stale directory means a message sent to a clinician who left two years ago.

Guaranteed delivery follows from the same problem. If a recipient cannot be reached electronically, a system that simply fails leaves the sender to discover the failure later, whereas one that falls back to another channel completes the communication. In clinical contexts an undelivered referral is a patient who does not get seen.

Care coordination extends this beyond one message. Managing a patient across a hospital, a specialist and a community provider requires a shared thread rather than separate exchanges, which is a different capability from secure point to point messaging and is where handovers between organisations usually break down.
